It is imperative that you complete the application in full and the following requirements are met prior to employment:

• Be 18 years old or older.
• Have or obtain (within the 1st 30 days of employment) current certification in Emergency First Aid and CPR.
• Have or obtain (within the 1st 30 days of employment) current certification in Elder Care Professional Fire Safety Course Training.
• Have current (within the past 12 months) documentation of TB (tuberculosis) screening with a negative result and a physical examination to assure that you free from communicable diseases and physically qualified to work the job (see form).
• FOR THIS TYPE EMPLOYMENT, STATE LAW REQUIRES A NATIONAL AND STATE BACKGROUND CHECK AS A CONDITION OF EMPLOYMENT. This is obtained with fingerprinting. If deemed unacceptable, your employment and/or offer for employment will be immediately terminated.  There are certain crimes which prevent a person from working in a Personal Care Home. 
• Include your employment history without gaps, for the past 5 years.  It is important to provide a complete account of your employment history as well as reference names and numbers to include dates of employment.
• Be in agreement to submit to a pre-employment drug screen.  We are a drug free employer, should you test positive for illegal drugs, your employment opportunity would be terminated.
